Becoming a patient at Magnolia takes about four steps.
- Reach out
Call (555) 014-2230 or submit the appointment request form. Our front office will call back within one business day to confirm.
- Send your records
We'll send a release form so your prior records and medication list arrive before your first visit.
- Your first visit
Plan for about an hour. Bring photo ID, insurance card, a current medication list (or the bottles), and any recent lab or imaging results you have.
- What to expect
We'll go through your history without rushing, set up any needed labs or screenings, and book you with the provider you'd like to see going forward.
